1. What we collect

When you use a Compliance Portal checklist or lead form — whether on this site or embedded on a partner website — we may collect:

  • Your email address, if you submit it to receive a checklist or follow-up.
  • Clinic details you provide: clinic type, state, services offered, ownership structure, and staffing roles.
  • Basic technical data such as the page you came from ("source"), used to route leads correctly.

We do not knowingly collect patient health information (PHI) or patient-identifiable data, and you should never submit it through the Service.

2. How we use it

We use the information you submit to generate your compliance checklist, store it so you can return to it, and to follow up with you about it. If you used the Service through a partner website (such as a clinic services platform), that partner may also receive and follow up on your submission.

3. Storage and processors

Submitted data is stored using Supabase, a third-party database and authentication provider. We may use Resend to send email notifications about new submissions. We do not sell your information to third parties.

4. Cookies

The dashboard uses cookies to keep you signed in. The public checklist and embed widget do not require cookies to function.

5. Your choices

You can ask us to delete your submitted information at any time by emailing us at the address below. We will remove it from our systems except where we're required to retain it by law.

6. Children

The Service is intended for business and professional use and is not directed at children.

7. Changes

We may update this Privacy Policy from time to time. Material changes will be reflected by updating the date above.
